Privacy & Financial Policy
The Client authorizes the Clinic to collect, retain and use any information about the Client, collected directly from the client or any other source, for assessing, the client’s credit worthiness, advising a default, or enforcing any rights under this agreement. The client authorizes the clinic to disclose any information obtained to any persons included a credit reporting agency for the same purposes. Where the client is a natural person the authorities above are consents for the purposes of the privacy act 1993. It is agreed that any treatment not paid for will be passed on to a debt collection agency. All costs of recovery will be incurred by the patient and will incur an additional $100 administration fee
Hamilton Family Dental reserves the right to charge $40 per failed appointment, especially when the appointment has been confirmed by the patient. We require at least 24 hours’ notice if you cannot attend or need to reschedule, otherwise normal charges will apply.
The pricing of treatment advertised is an indication of treatment costs, each final quote can only be given to the patient after consultation with the dentist. Prices quoted for services may be adjusted from time to time, and the patient hereby agrees to pay any such adjusted price, e.g. cost of goods increase, government surcharge increases, errors or omissions by Hamilton Family Dental or its representatives.
*Quoted prices for treatment will be honoured for up to one months from the issued date.
We accept Cash, Eftpos, Visa, Internet Banking, MasterCard, Q Card, Genoapay, Afterpay. laybuy, Partpay and Humm. **3% surcharge applies to all credit card transactions.
Payments are expected in full at the time of the dental treatment unless alternative payment options have already been discussed and agreed to by Hamilton Family Dental.
Accounts that are not cleared within 30 working days may be sent to debt collection. Should this action be taken, all costs incurred in the collection of overdue accounts in addition to the amount owing, will be payable by you the debtor.
There will be a $40 administration fee applied to all refund requests.
Winz Quote Refunds
Grants received by clients of Hamilton Family Dental through the Ministry of Social Development or Work and Income will only be refunded back to National Crown and Revenue. Under no circumstances will refunds be issued back to the client.
Transfer of Funds
Hamilton Family Dental will allow one client to transfer their remaining surplus funds to another client of Hamilton Family Dental for the express purpose of receiving dental treatment. Once the funds have been transferred, the client can make no future claims on the amount transferred.
All health information collected by the staff of Hamilton Family Dental is deemed to be confidential at all times.
Where the client has major health concerns or on medication, Hamilton Family Dental may request such information with the client’s general practitioner or other health professionals. Staff of Hamilton Family Dental may at times: Add to your health record during any services provided to you and use that information to provide appropriate care. Share relevant health information with other health professionals and organisations who are directly involved in your care
You as the patient have rights as outlined in the Code of Health and Disability Services Consumers’ Rights.
Client records will not be seen by individuals other than the staff of Hamilton Family Dental. Written patient records are to be always kept at the clinic under safe and confidential conditions. Patient records or copies of, or notes made from, will not be taken out of the clinic premises without the express verbal or written permission of the patient, HFD Management or unless required to by law or when required to by organisations directly involved in the patients care. Clients of Hamilton Family Dental may access their records when needed.
Any referrals cost to specialist/other dentist outside Hamilton Family Dental will be bear by the patient and will not be liable to Hamilton Family Dental or our Dentists.
